Hawaii – Misplaced Tax Documents

SALT Report 1853 – The Hawaii Department of Taxation is advising taxpayers that a courier service lost a bag containing various tax documents. State tax documents that were mailed or delivered to the Kona and Hilo Offices may be affected.

Kona Office: Business License Applications – Form BB-1, and Cancellation Forms – GEW-TA-RV-1, that were mailed or dropped off between September 24, 2012 and October 1, 2012

Hilo Office:  Various tax documents mailed between October 1 – October 3, 2012 or dropped off between October 3 – October 4, 2012

Taxpayers who believe they may be affected should contact the Hawaii District Office in Hilo at (808) 974-6321.
